How a CMAF output’s push destinations start: live (push as today) or held
(segments completed and reported, nothing pushed, buffered up to a cap until
the migration apply verb releases them). Local pull destinations are
unaffected. This is CONFIGURATION, not handover state — it says how the
output starts, which the workflow declares before any migration exists.
